Do you send an email to the address?
No. The checker opens an SMTP conversation with the receiving mail server and stops before the message body, so the mailbox owner never receives anything.
What does a 'risky' result mean?
Risky is reserved for genuinely ambiguous cases, such as role accounts like info@ or support@ where acceptance does not prove a human reads that mailbox. Catch-all domains are not lumped in: an ordinary SMTP check cannot tell one mailbox from another there, so most verifiers return 'risky' or 'accept-all', while VeriMailX resolves the individual mailbox to a definitive valid or invalid without sending an email to the recipient.
